Whole System Working

 

Why have we chosen this as a priority area of focus?

To ensure the care we provide is joined up and connected around people, we need to work as a whole system, keeping our people at the centre of everything and not letting traditional boundaries get in the way. Avoidable differences in healthcare need to be reduced as these are unacceptable. Consideration should be given to individual’s circumstances beyond their healthcare needs, working with our partners across the system to provide a holistic approach. We need to support change through building networks, collaborating with other anchor organisations and voluntary and community/social enterprise sector.

 

What do we want to see 10 years from now?

  • We have true devolved system leadership across traditional organisational boundaries.
  • We work together with partners in Grampian and beyond to ‘push’ in the same direction, agreeing and committing to delivering on shared actions.
  • We recognise and value the role all of our partners play and hold us and them to account.
  • Care is being delivered in the right place by the right people for the right purpose.
  • We see people as a ‘whole person’ in the context of where and how they live.
  • We support our regional and national colleagues in our role as a tertiary provider.

 

Will we know if we have achieved that?

  • We can demonstrate our shared goals and shared actions implemented.
  • We are explicit and clear on our priorities and what we will lead on and deliver.
  • We can demonstrate that we use our role as an advocate, ensuring the health perspective in all discussionswith our partners.
  • Transitions between pathways of care are seamless, ensuring continuity of care.
  • Transitions between organisations are seamless, with the sharing of appropriate information being easy and straightforward while important safeguards are still in place where needed.
